You need to bring the following documentation:

Medical renewal

Updated journal from your general practitioner, sundhed.dk/specialist doctor and medication card (FMK) since the last issuance
Passport or driver's license
Previous medical
Logbook
If you wear spectacles or contact lenses and the prescription has changed since the last issuance, a prescription from an optician or ophthalmologist must be brought with you

Initial examination

All medical records from your general practitioner (note that general practitioner notes/journals are not available on sundhed.dk)
All journal information from sundhed.dk and specialist doctors/private hospitals, as well as an updated medication card (FMK)
Passport or driver's license
If you wear spectacles/contact lenses, both spectacles and contact lenses shall be brought along with vision data(prescription) from an optician or ophthalmologist
